Eligibility

To qualify for medical marijuana through our office, you must:

  • Be 18 years old or above*.
  • Be a Florida resident
  • Have one or more of the listed qualifying conditions.

*Our office cannot certify pediatric patients below 18 years of age at this time. We apologize for any inconvenience.

Florida Residency

To prove your Florida residency, you must provide the following:
  • If you are a permanent Florida resident, you may provide any form of state-approved identification to prove residency.
  • If you are a seasonal or temporary resident, you may submit a copy of two of the following:
  • A monthly mortgage statement, deed, mortgage, residential rental or lease agreement
  • A proof of residential address from a parent, step-parent, legal guardian, or a person who the seasonal resident lives with along with a statement confirming the claim is true
  • A utility bill or work order
  • Mail addressed to you from either a financial institution (a checking, savings, investment account statement), or a federal, state, or municipal government agency no older than two months

  • You must be above 18 years old
  • We’re sorry, but we cannot certify pediatric patients below 18 years of age at this time

Qualifiying Conditions

Florida Statutes § 381.986 specifies that a patient must be diagnosed with at least one of the following conditions to qualify to receive marijuana or a marijuana delivery device:

(a)Cancer.
(b)Epilepsy.
(c)Glaucoma.
(d)Positive status for human immunodeficiency virus (HIV positive).
(e)Acquired immune deficiency syndrome (AIDS)
(f)Posttraumatic stress disorder (PTSD).
(g)Amyotrophic lateral sclerosis (ALS).
(h)Crohn’s disease.
(i)Parkinson’s disease.
(j)Multiple sclerosis.
(k)Medical conditions of the same kind or class as or comparable to those enumerated in paragraphs (a)-(j).
(l)A terminal condition diagnosed by a physician other than the qualified physician issuing the physician certification.
(m)Chronic nonmalignant pain.
Clause (k) “Medical conditions of the same kind or class as or comparable to those enumerated in paragraph” was written this way intentionally to allow certified physicians to use their medical judgment to certify patients whose medical conditions they deem would benefit from medical cannabis, even if it’s not explicitly listed. Therefore, if you do not see your condition listed above, it does NOT mean that you are disqualified!
