Certification Process

New patient
To qualify for medical marijuana use in the state of Florida you must be a Florida resident and have one of the listed qualifying conditions. Check your eligibility here (LINK to Eligibility tab). If you think you are eligible for medical marijuana and would like to schedule an appointment, proceed with the following steps.
2. Book an initial appointment
The State of Florida requires that all new patients visit a doctor in person to complete an initial health evaluation.

Once you request an appointment, you will receive an email to finish scheduling your appointment. Click on the confirmation link in the email to confirm your appointment. This must be done within one hour of requesting the appointment.
3. Sign up for OnPatient portal.
Once you confirm your appointment, click on the link in the pop-up widget to sign up for the OnPatient portal.
4. Pay initial consultation fee here
5. Fill out the intake questionnaire and review and sign the consent forms.
Prior to your appointment, log in to your OnPatient account and click on the check in link next to your appointment details. Fill out the intake questionnaire and review and sign all the consent forms.
6. Send proof of residency and medical records.
Prior to your appointment send a message to Dr. Romero through the OnPatient portal and attach proof of Florida residency (see eligibility page for details) and a copy of your medical records. Do not send these documents via your personal email account.
7. Attend an in-person appointment.
Visit Evergreen Cannabis Clinic for your scheduled appointment during which Dr. Romero will consult with you and talk about your medical condition and goals.
8. Apply for a medical marijuana card.
If approved, Dr. Romero will submit your recommendation to the Florida Department of Health Office of Medical Marijuana Use (OMMU) registry. You will then need to apply for the medical marijuana card.

Returning Patient
1. Pay follow-up visit consultation fee here
2. Book a video visit appointment.
Request a follow-up appointment here
or by signing in to your OnPatient portal.

4. Attend your telehealth appointment.
Within 30 minutes prior to your scheduled appointment, log in to your OnPatient account and click on the Start Video Visit link next to your appointment details.
